Position is located at Froedtert Hospital (Main Campus) Must have superior customer service and organizational skills, be able to prioritize work and work effectively with peers and hospital case management staff. Requires solid computer skills and ability to learn multiple applications to navigate and open referrals. Position is at an office within Froedtert Hospital and prioritizes hospice referrals from that hospital system, also helps coordinate and manage referrals connected to other providers and facilities. Highly desirable: Experience with Epic or other electronic health record systems, Intake experience, understanding of medical terminology, Hospice or Home Care experience/
Full-Time: Monday-Friday 8am-4:30pm.Froedtert Hospital - Wauwatosa
