Description & Requirements
Job Description

Interested in a dynamic, autonomous practice setting, working with a highly skilled and collaborative APP team? Consider joining our ambulatory care Cancer Center Infusion APP team. The Cancer Center Infusion APP team is responsible for overseeing and providing care for 50-75 patients per day (depending on the unit size) across five Infusion centers for Oncology and non-Oncology patients. Infusion APPs triage clinical concerns, such as acute treatment related symptoms, in real time and ensure continuity of care through collaboration and communication with physicians'/surgeons/APP colleagues, as well as other clinical and administrative staff. Together, the team supports timely patient treatment, works toward patients' goals of care, and promotes a positive patient experience and outcome. In addition to our infusion areas, APPs provide care at the three-bed Cancer Acute Care Clinic (CACC). There, they assess, diagnose, and manage acute oncology symptoms that can safely be treated in an outpatient setting. When a patient needs a higher level of care, APPs in the CACC coordinate a direct hospital admission when appropriate or facilitate emergency evaluation and admission. This role will require the ability to work in both the Infusion Centers and the CACC.
