Summary:

The Centralized Post-Acute Bed Placement Coordinator is responsible for coordinating placement of patients transitioning from acute care to post-acute services across the care continuum. The role serves as a clinical resource to support appropriate level-of-care determinations, timely transitions, and collaboration among care teams, patients, families, and community providers.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Associate's degree in appropriate clinical field required; bachelor's degree preferred.
  2. License/Certifications: Currentlicensure as a Registered Nurse, Physical Therapist or Occupational Therapist in the State of Maine required. Current Basic Life Support (BLS) certification is required or must be obtained within 30 days of start date. Case Management certification (CCM, ACM-RN, or equivalent) preferred.
  3. Experience: Three years' direct clinical experience required. Experience working in a skilled nursing facility or post-acute entity, discharge planning, utilization management, admission management, or post-acute care coordination preferred. Experience in a healthcare setting involving care transitions across multiple levels of care preferred.
  4. Additional Skills/Requirements Required: Knowledge of post-acute levels of care, including skilled nursing, inpatient rehabilitation, long-term acute care, home health, hospice, and related services. Knowledge of healthcare reimbursement and coverage requirements affecting post-acute services. Ability to review and interpret clinical documentation and patient care needs. Proficiency with electronic medical records and referral management systems.
  5. Additional Skills/Requirements Preferred: Knowledge of value-based care models and population health strategies. Knowledge of post-acute provider networks and community resources.

The pay range for this position is $40.06to $54.20 per hour.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
