Salary Up to $30.00 / Hour Overview Social Worker - Skilled Nursing Elderwood at Cheektowaga Help residents navigate change, preserve their independence, and remain connected to the people and resources that matter most. Elderwood at Cheektowaga is seeking a compassionate, resourceful Social Worker to support residents and families throughout the skilled nursing experience. In this role, you will address the psychosocial, emotional, and practical needs that influence each resident's well-being-from admission and adjustment through care planning, family communication, service coordination, and discharge preparation. This is an opportunity to become a trusted advocate for residents while working closely with Nursing, Rehabilitation, Activities, Dietary, medical providers, and other members of the interdisciplinary care team.
