Description Medicare Clinical Home Health Scheduler Activa Home Health - Stuart, FL Full-Time \| On-Site Starting at $19-21/hour (Negotiable Based on Experience) Join a Growing Leader in Home Health Care Activa Home Health, a proud member of the AccordCare family of companies, is seeking an experienced and highly organized Medicare Clinical Home Health Scheduler to join our team in Stuart, Florida. This critical role supports the coordination of skilled home health services throughout Martin and St. Lucie Counties, ensuring our patients receive timely, high-quality care in the comfort of their homes. We are looking for a scheduling professional who understands the fast-paced nature of Medicare home health operations, can effectively manage clinician schedules, and thrives in a collaborative healthcare environment. Responsibilities Schedule skilled home health visits for RNs, LPNs, PTs, OTs, STs, PTAs, and STAs Coordinate schedules based on physician orders, patient needs, and Medicare guidelines Manage daily clinician schedules and visit coverage Communicate with Clinical Managers, Intake, and field staff regarding admissions, schedule changes, and patient care Maintain accurate scheduling documentation within the EMR system Ensure continuity of care and timely visit completion Qualifications Required Minimum 3 years of Home Health experience Strong understanding of Medicare Skilled Home Health Scheduling OASIS knowledge required Experience scheduling nursing and therapy disciplines Excellent organization and communication skills Ability to multitask in a fast-paced environment Preferred Experience with Kinnser/WellSky EMR Why Join Us Growing and supportive healthcare organization Opportunity for advancement Team-oriented environment Meaningful work supporting patients in their homes This position is based in our Stuart, Florida office and requires successful completion of a Florida Level 2 background screening.
