Job Summary

The Social Worker - Ambulatory II is an integral member of the healthcare team, providing comprehensive psychosocial support and services to patients and their families. This role promotes mental, emotional, and social well-being through assessment, care planning, direct psychotherapy, resource coordination, advocacy, and ongoing evaluation. The Clinical Social Worker partners with patients and caregivers to address concerns, support coping, build trust, and develop individualized, patient-centered plans of care.

This position is based in the Patient and Family Resource Center at the NC Basnight Cancer Hospital and is an integral member of the Comprehensive Cancer Support Program (CCSP). The role works collaboratively within an interdisciplinary team that includes psycho-oncology and other supportive care services to meet the needs of patients and caregivers throughout the cancer treatment journey.

Duties

Description of Job Responsibilities: 30% assessments, triage, and care planning; 30% direct psychotherapy focused on caregivers; and 40% programmatic support for patient and caregiver services.

1. Assessments: Conduct comprehensive biopsychosocial assessments to evaluate patient and caregiver needs andidentifybarriers to overall functioning. Assess support systems, coping strategies, mental health conditions, and other psychosocial factors that may affect care.

2. Patient-Centered Care Plans: Collaborate with the Comprehensive Cancer Support Program team, patients, and caregivers to develop individualized care plans that reflect each person's unique needs, goals, and preferences. Plans may include referral to psychiatric or mental health services within Comprehensive Cancer Support, as well as other supportive care programs.

3. Caregiver Programming: Build on existing resources to develop caregiver-specific programming, including support group facilitation, educational workshops, caregiver advocacy, caregiver identification within the electronic medical record, visual cues for the care team, and development of internal and external resources.

4. Resource Facilitation: Connect patients and families with community resources, mental health services, support groups, and other relevant programs. Facilitate referrals for psychiatric evaluation, medication management, and coordination with other providers, while educating patients and families about available resources.

Other Information

Education Requirements:
* Master's Degree in Social Work from a program accredited by the Council on Social Work Education.
Licensure/Certification Requirements:
* Licensed as Licensed Clinical Social Worker (LCSW) in the state of North Carolina
Professional Experience Requirements:
* Three (3) years of broad clinical experience in a healthcare setting for patients with complex medical conditions required.
Knowledge/Skills/and Abilities Requirements:
* Knowledge of mental health conditions, diagnostic criteria, and evidence-based treatment modalities
Ability to collaborate effectively as part of an interdisciplinary team
Empathy, compassion, and cultural sensitivity
Ability to maintain confidentiality and adhere to ethical guidelines
