The Case Management Assistant (CMA) provides a broad range of support services to the transition guide nurses and the population health team. The CMA is an integral member of the care management team. The CMA works directly with patients, families, and post-acute providers to support patients after a hospitalization. These activities may include interviews with patients/families and telephone calls after a hospital stay. CMA will also be involved in various population health initiatives of which Suburban Hospital is involved. Overall assistance to patients/families will be supporting self-care habits in the community and relationships with primary care community providers. In this position, the CMA will have the opportunity to be part of an exciting multi-disciplinary team that will make a positive impact in healthcare across the continuum of care. Location: Suburban Hospital Bethesda, MD Shift: FT/Days Education: High School Diploma required. Associates Degree in Health Science or a closely related field preferred. Essential Duties & Responsibilities: * This position focuses on supporting patients in the hospital and home setting after a hospital stay. * This position will assist the hospital multi-disciplinary team, consisting of the patient's attending physician, medical specialists, diabetes educator, transition guide nurse, social worker, case manager, hospital nursing staff, family, community agencies and informal support networks, with communication, coordination and collaboration necessary for the success of the patient after leaving the hospital. * This position requires patient contact via in-hospital visits and post discharge telephone calls. * This position requires critical thinking skills, excellence in customer services, self-motivating capabilities, and the ability to work as a part of a team and independently * This position plays an important role in the workflow of various initiatives with the intent to support patients after leaving the acute hospital.
