The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The HIV Linkage Case Manager is responsible for ensuring individuals newly diagnosed with HIV, or those disengaged from care are rapidly connected to medical and supportive services,

This role focuses on timely engagement, reducing barriers to care access, and improving retention and health outcomes across the HIV care continuum.

Qualifications:

  • Strong knowledge of HIV care continuum and treatment guidelines, excellent communication and interpersonal skills, ability to engage vulnerable and diverse populations, knowledge of community resources and social determinants of health, proficiency in documentation systems and data tracking required.
  • The work environment is a combination of hospital-based, community-based, and field outreach work required.
  • Bachelor s degree in Social Work, Public Health or related field preferred, with 1-3 years of experience navigating patients in a medical setting.

Essential Functions:

  • Provide immediate outreach to individuals newly diagnosed with HIV,
  • Ensure linkage to HIV medical care within 30 days,
  • Coordinate initial medical appointments and accompany clients when appropriate,
  • Re-engage individuals who have fallen out of care,
  • Apply effective behavioral interventions as appropriate
  • Care Coordination
  • Conduct comprehensive needs assessments (medical, psychosocial, financial, and behavioral health),
