Responsibilities

Much more online at: https://summitoakshospital.com/ Position Summary: Responsible for assisting in discharge planning coordination with unit, social workers and Utilization Review staff. Serve as primary contact for District Attorney staff and Probate Courts for all patients for whom applications for court ordered mental health services are pending. Qualifications Education: Bachelor's Degree (Required) Completion of basic clerical or medical/clerical course. (Preferred) Related Work Experience: 1-3 years (Preferred) Specialized Skills and Knowledge: 1. Knowledge of general clerical procedures and excellent spelling skills required. 2. Knowledge of computers and software programs (i.e.WordPerfect, Lotus). 3. Effective oral and written communication skills. Ability to write neatly and legibly. 4. Ability to adhere to safety policies and procedures. 5. Ability to prioritize heavy workloads and ability to work in a fast-paced environment. 6. Ability todemonstratetact, resourcefulness,patienceand dedication. 7. Ability to recognize the importance of adapting to the various patient age groups (adolescent,adultand geriatric). 8. Ability to communicate effectively with patients and co-workers. 9. Ability to use good judgement and tomaintainconfidentiality of information. 10. Ability to work as a team player. 11. Ability to accept direction and adhere to policies and procedures. Essential Job Duties/Responsibilities: 1. Communicates inopendirectmannersharing information with team members. 2. Assist in discharge planning to ensure continuum of care following discharge. 3. Attendstreatment-planningmeetings, as is responsible and contributory member of the treatment team. 4. Documents intervention with patient and family in patient medicalrecordona timelyand professional basis. 5. Provides andmaintainspositive relationships with community agenciesin order toprovide the mostappropriate assistanceto patients, and to help foster effective working relationships between outside agencies and hospital staff. 6. Communication skills are employed to that patientdemonstratesfeelings of trust and rapport. 7. Recognizes learning needs andutilizesresources to meet such. 8. Complete paperwork for all transfers and communicate with Excel staffregarding timeframe of transfers. 9. Recognize andparticipatein ensuring the provision of quality and responsive patient care. 10. Maintain confidentiality of patients' medical information. 11. Complete and process discharge papers and assemble the patient chart for medical records. 12. Assistpatient/family with discharge process. 13. Answer patient/family calls in a polite andtimelymanner. 14. Maintain a professional relationship with staff members, providing support as requested. 15. Assistthe staff withlocatingcharts and returning charts to theappropriate areawhen finished. 16. Participate on committees asassigned, andmeet all assigned committeeobjectives. 17. Participate in cost containment for the program area by adhering to cost containment procedures, reporting anydiscrepanciesand recommending changes which will increase cost containment goals. 18. Adhere to facility standards concerning conduct, dress,attendanceand punctuality. 19. Attend all mandatory facility in-services and staff development activities as scheduled. 20.
