At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks in the West Hollywood, CA area. Support members who are experiencing/ are at risk of experiencing homelessness with managing their own health. Assist in motivating behavioral changes in patients to improve health outcomes in members through education, peer support, and the relaying of shared experiences. The Lead Peer Navigator will both provide direct care as described above and mentorship and guidance to a team of up to 5 other Peer Navigators working for Healthcare in Action.

Responsibilities:

  • Build rapport with members by communicating personal experiences with a goal of increasing the likelihood of positive behavior changes.
  • Coach members to create and implement strategies that minimize risks associated with the identified common health conditions and behaviors Connect members to appropriate programs to address barriers to care and to enhance compliance
  • Link members to local, county and state services Follow up with members and serve as a member advocate
  • Accompany members to medical and social services appointments that fall outside of the street medicine unit
  • Introducing systems to promote self-management & self-efficacy
  • Update members' Plan of Care after every street medicine encounter Document all required information in the care management system during phone calls with members Complete and submit a Monthly Audit Checklist
  • Schedule encounters based on member's acuity level using the Healthcare in Action electronic health record system
  • Provides leadership and mentorship to Peer Navigators on the Healthcare in Action team
  • Sr / Lead Role (prioritizes assignments, provides assistance, scheduled work, but does NOT directly supervise

Qualifications:

Preferred Education:

  • Associate degree or equivalent experience
  • 0-3 years of related experience. Performs work under direct supervision. Handles basic issues and problems and refers more complex issues to higher-level staff . Possesses beginning to work knowledge of subject matter.
