Overview

Responsibilities:

  • Assess patient data and perform interview, physical examination and begin discussing goals of care within prescribed guidelines and instructions of Physician.

  • Maintain timely consult schedule.

  • Provide patient and family education to assist patient and family to understand role of palliative care in current health situation.

  • Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required.

  • Record physical findings, and formulate plan and prognosis, based on patient condition.

  • Coordinate care with Physician and other health professionals; preparing comprehensive patient care plan.

  • Discuss health care plan and goals of care with Physician partner.

  • Consult with partnering physician and refer to specialized health resources for treatment and evaluation as needed.

  • Assists in developing care protocols and processes.

Requirements:

  • Graduate of an accredited nursing program.
  • Graduate of an accredited masters of nursing program.
  • Meets the requirements determined by the State of Illinois to perform duties defined as those of a Nurse Practitioner.
  • Hold valid RN license in Illinois

  • Hold valid APNP license in Illinois

  • Approved CPR or BLS certification required within 90 days

  • Other certification per unit requirements
