Summary:

Supervise a team of Community Health Workers across the state embedded in outpatient clinics that provide connections to community resources, basic health education and access to care navigation. Develop/maintain a coaching model used to track employees performance (quality, production, attendance, etc.) and development. Understand program performance statistics and reporting systems available and work closely with the Manager/Director to apply them in managing service levels and staff performance. Be available to the staff to answer questions, take escalated calls, and monitor performance measures, etc. Motivate and encourage representatives through positive communication and feedback.

A hybrid work schedule available for this role with the expectation that the Supervisor have a standing in-person presence and make an effort to visit team members for in-person events, meetings, and touch-points. Some travel is required.

Responsibilities:

Presbyterian is seeking a Supervisor for Community Health, a dynamic leader responsible for recruiting and developing highperforming teams, driving quality outcomes, and implementing programs that address social determinants of health, substance use, and health promotion. This role oversees performance management, enforces organizational policies, and partners with delivery sites to integrate continuous quality improvement into daily workflows. The Supervisor monitors resource gaps for patients, interprets data to guide team and individual development, and provides feedback that supports innovative peerteam processes. Additional responsibilities include updating community and behavioral health programs to align with evolving healthcare trends, mentoring staff to deliver exceptional customer service, collaborating across crossfunctional departments, engaging community health workers and peer support specialists to meet departmental goals, and traveling to field sites.

This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)

Qualifications:

  • Bachelors degree in a related field,

  • 6 years of additional experience can be substituted in lieu of degree. Plus 3 years experience as a Community Health Worker, Case Manager, Peer Support Specialist, Care Coordinator, or related position.

  • 2 years supervisory experience preferred.

  • Travel required.
