About Duke HomeCare & Hospice

Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

The Senior Director, Home-Based Care provides strategic and operational leadership across home health, hospice, and infusion services, ensuring the delivery of high-quality, patient-centered care in the home setting. This role partners with interdisciplinary leaders to drive clinical excellence, regulatory compliance, and sustainable growth while fostering a culture of collaboration, innovation, and compassionate care. Ideal candidates bring advanced clinical leadership experience, strong regulatory expertise, and a passion for developing teams and improving outcomes across complex, multi-service operations.

JOB SUMMARY:

The Sr. Director, Home-Based Care provides strategic oversight and operational leadership for all home-based care programs, including but not limited to; Home Health, Hospice, and Infusion which includes Nursing, Pharmacy and Distribution services. This role ensures the delivery of high-quality, patient-centered care in the home setting, emphasizing safety, comfort, and dignity. The leader will integrate clinical operations across service lines, coordinate interdisciplinary teams, and champion compliance with federal, state, and accreditation standards. By fostering innovation and collaboration, this position drives clinical excellence, regulatory adherence, staff development, and financial sustainability within a home-based care environment.

Clinical Responsibilities




  1. Develop and implement integrated clinical strategies for home health, hospice, and home infusion nursing and home infusion pharmacy programs.
  2. Develop and foster effective collaboration across service lines to ensure integrated clinical care.
  3. Monitor and elevate clinical outcomes, patient satisfaction, and implement corrective actions.
  4. Direct business line directors/operators, ensuring consistent standards of care.
  5. Foster collaboration among nursing, therapy, pharmacy, and social work/bereavement teams while elevating staff engagement and satisfaction
  6. Support clinical education, competency programs, and leadership development.
  7. Ensure compliance with infection control, medication management, and safety protocols.
  8. Promote a culture of compassion, dignity, and respect for patients and families.
  9. Ensure seamless transitions of care between service lines and external providers.



Operational Responsibilities




  1. Lead initiatives for growth, innovation, and service expansion in alignment with strategic objectives
  2. Provide administrative directions to management staff (planning, decision-making, professional development).
  3. Optimize staffing models, scheduling, resource allocation, and operational efficiency
  4. Coordinate resource needs, review budgets, and prepare variance analyses.
  5. Collaborate with finance to develop and manage budgets, cost controls, and reimbursement compliance.
  6. Coordinate with both DHCH entity and Duke Health regulatory, compliance, risk management, and patient safety operations (as a governance function).
  7. Maintain readiness for audits and surveys, and lead corrective action plans with a focus on staff and patient safety and clinical standards



Both Clinical & Operational




  1. Align clinical operations with organizational goals, quality benchmarks, and regulatory requirements.
  2. Ensure adherence to CMS Conditions of Participation, Joint Commission standards, state licensing regulations, and other pertinent regulations.
  3. Collaborate with accreditation, compliance, and performance Improvement programs.
  4. Oversee and optimize day-to-day clinical and administrative operations.
  5. Direct activities with managers and administrators to ensure high-quality, efficient clinical services. Perform other related duties incidental to the work described herein.



Qualifications

Education:



  • Completion of an accredited bachelor's degree in a clinical field such as Nursing, Pharmacy, Physical Therapy, Occupational Therapy, Speech Therapy or a related field; advanced degree (MSN, MBA, or MHA) preferred.



Licensure:



  • Current North Carolina licensure or ability to obtain as required by professional discipline.



Professional Work Experience:



  • Minimum 7-10 years of progressive leadership experience in Home Health, Hospice, and/or Infusion.



Knowledge Skills & Abilities:



  • Strong knowledge of CMS regulations, accreditation standards, and reimbursement models.
  • Proven ability to lead multi-site, multi-service clinical operations.
