About Duke HomeCare & Hospice Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC. The Senior Director, Home-Based Care provides strategic and operational leadership across home health, hospice, and infusion services, ensuring the delivery of high-quality, patient-centered care in the home setting. This role partners with interdisciplinary leaders to drive clinical excellence, regulatory compliance, and sustainable growth while fostering a culture of collaboration, innovation, and compassionate care. Ideal candidates bring advanced clinical leadership experience, strong regulatory expertise, and a passion for developing teams and improving outcomes across complex, multi-service operations. JOB SUMMARY: The Sr. Director, Home-Based Care provides strategic oversight and operational leadership for all home-based care programs, including but not limited to; Home Health, Hospice, and Infusion which includes Nursing, Pharmacy and Distribution services. This role ensures the delivery of high-quality, patient-centered care in the home setting, emphasizing safety, comfort, and dignity. The leader will integrate clinical operations across service lines, coordinate interdisciplinary teams, and champion compliance with federal, state, and accreditation standards. By fostering innovation and collaboration, this position drives clinical excellence, regulatory adherence, staff development, and financial sustainability within a home-based care environment.
