Requirements

The ideal candidate will possess the following qualifications:



  • Masters Degree in social work, psychology, rehabilitation, activity therapies, counseling or education and 3 years mental health direct care experience; OR


A bachelor's degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, education,


or be a registered nurse, and 5 years mental health direct care experience, 2 of which shall include supervisory experience; OR


A bachelor's degree in nursing and 3 years mental health direct care experience.



  • 1 year management experience
  • Available to work: Monday-Friday 9am-5pm; 8am-4pm; plus on-call as needed for team support


Skills needed:



  • Knowledge of human services principles, best practices, and evidence-based interventions relevant to the program's focus area (e.g., mental health, homelessness, youth services).
  • Knowledge of community resources, services, and support networks available to program participants, including social service agencies, government programs, and community-based organizations.
  • Strong problem-solving skills, with the ability to identify challenges, develop solutions, and make informed decisions in a fast-paced and dynamic environment.
  • Excellent organizational skills, with the ability to manage multiple priorities, meet deadlines, and coordinate program activities, resources, and logistics effectively.
  • Flexibility and adaptability to respond to changing needs, priorities, and circumstances, adjusting program strategies and approaches as necessary to achieve desired outcomes.

Blended Case Management Program Manager Opportunity in Bucks County!
Make an impact by leading services that support individuals ages 5 and up.

As Program Manager, you'll oversee daily operations, guide staff, coordinate program activities, and ensure highquality, clientfocused care.


The Case Management Program Manager will:



  • Oversee community-based Case Management Program and team which assists consumers by linking them to services and resources.
  • Oversee daily operations of the Program, ensuring alignment with organizational mission, strategic goals, and regulatory standards.
  • Review of financial guidelines and productivity of the case management team.
  • Provide direct supervision, support, and training to case managers (up to 9) and the program administrator.
  • Support and guide in Crisis Response
  • Support staff in developing individualized service plans and addressing complex client needs through case consultation and coaching.
  • Identify opportunities for program growth, innovation, and funding.
  • Support in the development and management of program budgets, revenue analysis, overseeing billing processes and ensuring timely collection while working with the AR team.
  • Perform other duties as assigned.



Earn $30.77 per hour.
