Position Summary

The Pharmacy Tech, Medication History is responsible for the medication history process for admitted patients. This position demonstrates efficacy in evaluating documented medication histories using structured patient interviews and standardized forms and processes. Furthermore, this position is responsible for ensuring medication lists are updated for all high-risk patients. Their role is designed to improve accuracy of prior admission medication histories and optimize patient outcomes through interdisciplinary medication management in an inpatient acute care hospital setting.

1. Performs thorough patient medication histories and documents accurately into the electronic medical record.
2. Able to perform the duties and functions of a dispensing Pharmacy Technician.
3. Participates in quality assurance programs by maintaining medication history data and reporting any significant medication errors.
4. Demonstrates good customer services skills with colleagues and communicates effectively with other departments.
5. Able to be at work and be on time.
6. Able to follow company policies, procedures and directives.
7. Able to interact in a positive and constructive manner.
8. Able to prioritize and multitask.

2. Displays attention to detail when documenting prior to admission medication history.
3. Educates oneself and trains/ educates students and new staff.
4. Demonstrates leadership skills that positively contribute to the Medical Center.
5. Provides pharmaceutical care in accordance with law and regulations.
6. Supports the direction and initiatives of the Department of Pharmacy.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
