Position Overview: Social Worker

Elevate your career as a Social Worker and become an integral part of our compassionate healthcare team, dedicated to providing exceptional psychosocial support to patients and their families. In this impactful role, you'll conduct thorough assessments, collaborate closely with medical professionals, and implement transformative interventions that enhance patient care and outcomes. Your expertise will help identify and overcome barriers to care, ensuring that our patients receive the resources they need in compliance with JHACH policies and regulatory standards.

Key Responsibilities:



  • Conduct comprehensive psychosocial assessments to address the social and emotional needs that influence patient treatment and recovery.
  • Perform risk assessments, including evaluations for suicide risk, to provide informed safety recommendations to the medical team.
  • Facilitate referrals and connect families with valuable community resources, enhancing care coordination and reducing psychosocial stress.
  • Deliver therapeutic interventions such as crisis intervention, grief counseling, and psychoeducation, equipping patients with healthy coping strategies.
  • Collaborate with the healthcare team to develop tailored care plans that tackle psychosocial barriers, preventing discharge delays.
  • Maintain accurate electronic documentation and data collection, ensuring adherence to regulatory requirements.
  • Communicate effectively with team members regarding patient needs, interventions, and insights during meetings and rounds.


Qualifications:



  • Master's Degree in Social Work.
  • Current licensure as a LMSW or LCSW-C.
  • Completion of JHACH's intern program or a comparable internship in a healthcare environment.
  • Exceptional listening, verbal, and written communication skills.
  • Proficiency in electronic medical record documentation.


Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
