POSITION SUMMARY
The Community Resource Navigator (Navigator) provides school-based case management services for Latino children and their families in the Suburban Ramsey school district: Mounds View (ISD 621), NSP-M-O (ISD 622), Roseville Area Schools (ISD 623), and White Bear Lake (ISD 624). The Navigator develops and coordinates client and family case plans, provides resource navigation and referrals, and assists families to create positive academic and social outcomes. They also participate in agency activities that enhance the capacity of CLUES to provide quality services.

Location: CLUES St. Paul, MN Office

Position Type: Regular

Schedule: Full-Time. Monday - Friday Office hours, with some evening & weekend hours.

Estimated Pay: $22.00 - $24.00 per hour (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide school-based case management services to Latino children and their families, including conducting home visits.
  • Develop, assist, and monitor an action plan/case plan with each family receiving services.
  • Provide information, outreach, and referral services to participating families.
  • Coordinate and facilitate accessibility to needed community services.
  • Assist parents in utilizing school and community resources to help children maximize their educational abilities. Such as promoting parental involvement with the school to create positive academic and social outcomes for the children.
  • Safely transport clients to and from activities and/or appointments as needed by personal motor vehicle.
  • Maintain timely and appropriate documentation and charting as required by the Suburban Ramsey Collaborative Safe Schools/Healthy Students Initiative and CLUES policies.
  • Assist program staff in preparing program reports and evaluations
  • Participate in weekly case management meetings, team meetings, all staff meetings, and other meetings as assigned.
  • Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations, and ethical standards.
  • Promote the mission, operating values and agency services of CLUES within the community and cultivate relationships with potential service collaborators and partners.
  • Carry out all other duties as assigned by supervisor.

QUALIFICATION REQUIREMENTS

Education, Experience & Key Abilities

  • Bachelor's degree in social work or related field preferred.

  • One-year experience working with children and families required.


  • Case management/social work experience preferred.

  • Bilingual in English and Spanish with cultural sensitivity and awareness required.
  • Working knowledge of resources in the community required.
  • Strong computer skills required. Experience with client charting software such as Procentive preferred.
  • Comfortable working with people from a diverse range of ethnic and social economic backgrounds.
  • Excellent communication skills and ability to respond tactfully and respectfully to sensitive client situations required.
  • Position requires driving and transporting clients at times in personal vehicle. Therefore, having and maintaining access to a motor vehicle, valid driver's license, proof of automotive insurance, and a good driving record and driving history is required.

Language Skills


  • Effective communication in English and Spanish, both verbally and in writing required.
  • Ability to read, analyze, and interpret written materials, maintain accurate documentation, and respond appropriately to questions and requests.


Transportation

  • Must have a motor vehicle for independent transportation, valid driver's license, proof of automotive insurance, and a good driving record. Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.


Computer Skills

  • Proficient in Microsoft Office (Word, Excel, PowerPoint) and other common software (e.g., database, reporting, client management systems, etc.).
  • Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties.



WORKING CONDITIONS
Background check

  • Must pass a criminal background check, driving record check, and MS-604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.
