Job Summary

Service Source partners with HOST (Department of Housing Stability) to conduct outreach and recruitment for Denver Day Works (DDW), a job readiness program for individuals experiencing homelessness. DDW provides daily work opportunities, support services, and job readiness programming that may lead to employment. The DDW Case Manager helps participants meet basic needs and connects them to resources like food, shelter, healthcare, and public benefits. This grant-funded position includes fieldwork to assess participants' strengths and barriers to employment.

Primary Duties

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These duties and responsibilities will be rated on the Annual Performance Review.

  • Conduct outreach and recruit individuals experiencing homelessness for the Denver Day Works (DDW) program in partnership with HOST.
  • Support participants through initial and ongoing assessments to identify and address basic needs that reduce barriers to employment.
  • Connect participants to essential resources such as food, shelter, clothing, and healthcare based on their individual needs.
  • Assist participants with public benefit applications and conduct assessments like VI-SPDAT to evaluate housing and resource vulnerability.
  • Engage with participants in the field to understand their work strengths, limitations, and resource navigation needs.

Additional Responsibilities

  • Providing job readiness programming that may lead to employment.
  • Linking participants to internal or external providers for SSI/SSDI application assistance or representation.
  • Tracking participant progress and maintaining accurate documentation.
  • Coordinating with other service providers to ensure continuity of care.
  • Supporting participants in navigating housing and employment systems.
  • Maintaining timely and accurate data entry, service notes, and documentation in systems such as HMIS and E-Logic.
  • Building strong relationships with participants and community partners, including property management and housing authorities.
  • Upholding confidentiality of all participant information and materials.
  • Remaining culturally attuned, sensitive, and compassionate in all interactions.
  • Perform other responsibilities as assigned.

Qualifications: Education, Experience, and Certification(s)

  • 2 years of experience in case management required.
  • High school diploma or General Education Development (GED) preferred.
  • A valid driver's license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR) required.

Knowledge, Skills, and Abilities

  • Proficient in Microsoft Office, especially Excel, Outlook, and Word.
  • Strong communication skills, both written and verbal, with excellent grammar and reasoning.
  • Creative problem-solving abilitieswith a practical, solution-focused mindset.
  • Maintains confidentialityand handles sensitive information with discretion.
  • Technologically proficient, including use of smartphones, tablets, laptops, and remote software.
  • Applies evidence-based practicessuch as Motivational Interviewing, Harm Reduction, Trauma Informed Care, and Mental Health First Aid to support participants.
  • Effective communicatorwith both participants and staff across various settings.
  • Independent project managementskills with the ability to track and report outcomes.
  • Strong commitment to the agency's mission and goals.
  • Demonstrated analytical skills, including interpreting documents, performing mathematical calculations (e.g., percentages, geometry, budgeting), and solving practical problems using various instruction formats.
