Comagine Health partners with the Oregon Health Authority (OHA) to provide statewide eligibility determination, service coordination, medical appropriateness reviews, and transition planning for individuals diagnosed with serious and persistent mental illness (SPMI).

Our goal is to support adults with SPMI to live in the least-restrictive settings possible by ensuring that necessary and desired supports and services are in place. The Home and Community Based Coordinator facilitates timely and appropriate discharges and care transition support services through collaboration with providers, community service agencies, and other stakeholders.

What you'll be doing for us:



  • Facilitates timely and appropriate discharges and care transition support services through collaboration with providers, community service agencies, and other stakeholders
  • Conduct brief functional needs assessments, develops service plans and provides service coordination
  • Provide assistance and support to the Oregon Behavioral Health Support Program and other Comagine Health staff
  • Maintain and distribute information about the availability of community resources
  • Assist and support Medicaid consumers during the care transition process
  • Participate in discharge planning, service coordination, and implementation of the service plan
  • Participate in and facilitate consumer and/or provider education
  • Coordinates initiatives, tracks, and monitors progress, and sets timelines for delegated assignments
  • Coordinates, organizes, and maintains effective filing system and performs complex data entry and data management
  • Ensures all records and business sensitive information are developed, distributed, handled, retrieved, and archived
  • Develops and maintains documents, reports, templates, etc.
  • Receives and processes requests for service from any source (via phone, fax, SFTP or mail) and coordinates with team
  • Performs data entry into systems (both external and internal) for requests, services provided, authorizations, etc.

Required Skills

  • Knowledge of the State of Oregon: Home and Community Based Services (preferred)
  • Strong MS Office Suite proficiency and familiarity with database software programs
  • Strong organizational skills
  • Excellent oral and written communications skills
  • Excellent interpersonal and problem-solving skills
  • Ability to organize and coordinate multiple simultaneous tasks in a team environment
  • Makes decisions in a timely manner

Required Experience

  • Bachelor's degree in a related field; Equivalent combination of education and/or work experience in related field may be substituted
  • Qualified Mental Health Associate (QMHA) Certification required
  • 2+ years of clinical experience
  • 1+ year of care coordination experience (preferred)
