Position Summary CCA Primary Care provides primary care for complex patients with disabilities and frail elders in their home, virtually, or at one of our clinic locations. We are an interdisciplinary care team consisting of APCs, physicians, behavioral health clinicians, nurses, medical assistants, rehab, and administrative staff. The Clinical Support Coordinator provides day to day administrative support to the care team including but not limited to coordinating outgoing referrals to specialists and VNA agencies, scheduling patient appointments, scanning and filing documents, front desk support, sending correspondence, answering phone calls and any other administrative tasks needed as determined by the care team to ensure patients receive timely and high-quality care. Supervision Exercised: No Essential Duties & Responsibilities
Working Conditions Standard office conditions. Required Education High School Diploma/GED Desired Education Associates degree Required Experience 1-2 years related experience either in a healthcare or medical office setting Required Knowledge, Skills & Abilities

Compensation Range/Target: $21.61 - $32.42 Commonwealth Care Alliance takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level.
