Summary: Under the general supervision of the Manager Director of Cardiology and the Medical Director of the Cardiac Pulmonary and Vascular Rehabilitation Programs evaluates provides physical assessment and performs all other duties pertaining to an exercise prescription for cardiac pulmonary and vascular patients within the Academic Medical Center. All essential functions performed in this position reflect the age specific needs of the patients. Responsibilities: Performs comprehensive evaluation and physical assessment of new participants for initial assessments and/or stress testing obtains patient history pertinent diagnostic data and risk factor information from the medical records. Reviews assessments made by Cardiac and Pulmonary Care Specialists when goal planning and ongoing assessment. Provide optimal care delivery by taking into consideration cardiac pulmonary and vascular history risk stratification and comorbidities. Target psychosocial nutritional tobacco cessation and exercise interventions by partnering with Care Specialists other members of the cardiac/pulmonary rehab team members of the patient*s healthcare team and patient/family taking into consideration risk stratification and comorbidities. Creates exercise plan for patients eligible for cardiac pulmonary and vascular rehab as well as patients in self-pay Maintenance and Primary Prevention programs. Performs and/or reviews stress tests for purpose of establishing safety for exercise and target heart rates of cardiac and vascular rehab patients. Conducts supervised exercise sessions for patients evaluating hemodynamic oxygen saturation electrocardiographic and symptomatic response to exercise and assesses appropriateness of responses to the prescribed exercise intensity. Refers to and communicates with additional healthcare providers when needed. Reviews patient responses to exercise prescription with Cardiac and Pulmonary Care Specialist to ensure appropriate and optimal exercise prescription. Modifies exercise prescriptions when changes in safety comorbidities medical status medications or environmental conditions occur. Assesses psychosocial adjustment to illness including depression return to work family adjustments or any significant social dysfunctions that may affect a full rehabilitation. Makes referral to department*s licensed mental health provider or works with other agencies on patient*s behalf. Provides stress management strategies such as relaxation meditation and yoga both individually and/or in groups. Provides individualized nutritional strategies for healthier eating. Makes referrals for nutritional weight loss and diabetes counseling. Conducts group stretching and patient education classes that include ongoing revision and evaluation of materials. Collects data on all patients to perform 30-day reassessments which evaluates progress towards established goals and identifies any additional target interventions or education. Per Medicare guidelines obtains medical director*s consent on all treatment plans and assessments. Develops a comprehensive discharge plan which includes discussion and review of outcomes with patient and uploading outcomes to the AACVPR Registry. Acts as a change agent by identifying opportunities to improve adherence and compliance to reduce readmissions through team collaboration problem resolution and follow up. Participates in departmental quality improvement projects hospital committees and/or AACVPR community. Follows routine standing orders and departmental emergency policies which include the initiation of Advanced Cardiac Life Support procedures. Receives and transcribes physician*s or advanced practice provider*s (APP) orders. Maintains correspondence with referring physicians or APPs . Notifies referring physicians or APPs of any untoward events. Intervenes when necessary and appropriate in situations requiring nursing assessment and/or intervention. Initiates emergency procedures per protocols. Responds to all emergencies during the program hours and prepares the patient for transport to and communicates with the next level of care. Prioritizes patient care by assisting Cardiac and Pulmonary Care Specialists in their roles to appropriately assess patients experiencing a change in clinical status medical issue or emergency. Assists co-workers in the interpretation and significance of any electrocardiograph hemodynamic or physiological changes that the patient may be experiencing. Serves as a resource for the other professionals who work closely with the Cardiac Pulmonary or Vascular Rehabilitation population. Assists manager and/or supervisor in developing and implementing quarterly competencies and code reviews for all staff. May assist manager supervisor or lead in the precepting or mentoring new hires students and volunteers. Assists with maintenance of emergency supplies and crash cart. Maintains quality assurance safety environmental and infection control in accordance with established hospital department policies procedures and objectives. Performs charge duties as required. Performs other related duties as required. Performs all essential functions with consideration of the age-specific needs of the geriatric adult and adolescent patient. Fosters patient and family centered care and healthcare services in accordance with the Brown University Health and The Miriam Hospital*s mission vision values. Other information: BASIC KNOWLEDGE Graduate of an accredited School of Nursing with current license to practice as a Registered Nurse in the State of Rhode Island. Conforms to the Code of Professional Nurses. Understands the provisions of the Nurse Practice Act of the state of Rhode Island. Responsible for maintaining a current knowledge of contemporary professional nursing practice within the legal framework of the Nurse Practice Act. Knowledge of exercise physiology and exercise prescription required. Master*s Degree in Exercise Physiology with a cardiopulmonary specialization preferred. Experience should include EKG and rhythm strip interpretation. American College of Sports Medicine Exercise Specialist certification preferred. Current Advanced Cardiac Life Support certification required; may obtain within six months of appointment. Demonstrated knowledge and skills necessary to provide care to patients throughout the life span with consideration of aging processes human development stages and cultural patterns in each step of the care process. EXPERIENCE: Six months of Cardiology experience required. Six months of experience in Cardiac Rehabilitation preferred. WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: Healthcare environment such as acute care hospitals physicians* offices and ancillary healthcare enterprises. Works with medical equipment and has direct patient contact. Any potential hazards are minimized or nullified by adherence to established safety procedures and precautions including use safety equipment and protective clothing. Prolonged sitting standing and walking. May be required to lift/turn patients and equipment. Repeated bending squatting stooping. SUPERVISORY RESPONSIBILITY: May be assigned technical supervisory responsibility for up to eight FTEs. Supervision limited to assigning work scheduling providing technical oversight training guidance and counsel.
