What You Need to Know

The Quality Improvement Specialist is responsible for monitoring, analyzing and doing in depth review of key quality metrics foundational to AccentCare's ability to deliver high quality, safe care to our patients and achieve progress on the Hospice Quality Reporting Program*. Ensures compliance with state, federal and accreditation standards. This position is a member of the quality management team and responsible for providing expertise in quality improvement, performance improvement, and change management. They promote commitment to the corporate and subsidiary standards, policies, and procedures while teaching, coaching, and providing consultation on measure development and methodology, goal setting, data analysis (including root cause and risk assessment), problem solving, corrective action, and evaluation of the effectiveness of action taken The position guides leaders through project work, facilitation, consultation, coaching, skill building/training and reporting in a complex community based healthcare environment. They work closely with team members in the Quality, Clinical Operations, and Compliance departments as collaborators, supporting root cause analysis facilitation of safety events, and coaching teams on implementing change. They promote a culture of diversity, equity, inclusion and belonging through interactions and commit to decrease systemic healthcare disparities

  • Drive safe care delivery at the agency level by assisting agencies in maintaining standardized clinical and
  • documentation procedures that provide for an independent, impartial, and objective system of surveys, clinical audits and reviews for use by the Operations and AccentCare managers as appropriate; determines best metrics for evaluating success and monitoring cadence for agency success.
  • Teach, coach, and consult to promote adherence to applicable legal requirements, standards, organizational policies and procedures.
  • Demonstrates effective strategies for agency adoption/execution for healthcare accreditation and program requirements.
  • Applies project management fundamentals to drive improvement projects from identifying the problem,
  • generating interventions, and coordinating methods for monitoring.

Qualifications

Be the Best Area Hospice Quality Specialist You Can Be

If you meet these qualifications, we want to meet you!

  • Holds a license in healthcare (e.g., nurse, physician, social work)
  • Certification or advanced education in Healthcare Quality or experience that demonstrates expertise in performance improvement & regulatory standards in healthcare
  • Minimum 5 years of health care experience
  • Minimum 2 years of Hospice or Home Health experience
  • Knowledge of regulatory standards for the home health and/or Hospice industry
  • Excellent knowledge base of quality/performance improvement processes and tools
  • Systems knowledge and experience in identifying root causes and developing reliable and safe
  • systems/processes
  • Able to effectively work with and communicate with staff in all levels of the organization
  • Strong project/process management skills
  • Ability to create complex documents, spreadsheet analysis models, and advanced graphics
  • Ability to work in a fast paced, multi-task environment with competing priorities
  • Able to lead group discussions with medical clinicians, administrators, and stakeholders
Required Certifications and Licensures:
  • Holds a license in healthcare (e.g., nurse, physician, social work)
  • Certification or advanced education in Healthcare Quality or experience that demonstrates expertise in performance improvement & regulatory standards in healthcare
