865 Stone Street, Rahway, NJ 07065 Job Overview: The RWJ Rahway Community Health Navigator for Chronic Disease will ensure patient access to healthcare and community services by interacting with Rahway s patients with chronic health conditions to address barriers to care, provide guidance and healthcare education, and assist in the coordination of care in order to maintain a healthy lifestyle and meet identified health goals. This role acts as a liaison between patients, healthcare providers, and community resources to ensure continuity of care and improved health outcomes. Qualifications: Required: High school graduate or equivalent required Basic computer skills required Valid driver s license required Annual competency assessment and goal review required Successful completion of a hospital approved Community Health Worker, or comparable, formal training program is expected upon hire Preferred: Electronic medical record (EMR) experience preferred Knowledge of medical terminology preferred Scheduling Requirements: 8:00am/8:30am to 4:30pm/5:00pm Monday-Friday Full-Time Essential Functions:
