Job Summary
Responsible for direct services for Friendship House and Cottage programming, and overseeing staff for 1915i Medicaid services, including developing and implementing Individual Service Plans (ISP) for program participants and providing supportive housing and living interventions to enhance life skills and independence of our participants. Coordinate required supports for Friend residents with Employment Source staff and community partners.

Primary Duties

* Provide ongoing life skills development including problem solving and skill development to aid in the Friends' process of improving independence and quality of life (provide specialized and targeted interventions as needed) with decreasing support at the Cottage level.
* Coordinate case management and supports for Friends and provide mentoring/training support for student/young professionals.
* Attend/facilitate/jointly develop Individual Service Plans for FHF Friend residents with parent/guardians to ensure goals are achieved and steady progress is made.
* Oversee 1915i Medicaid services to include, Community Living and Support and Supported Employment services to ensure the program is aligned with the goals of our mission and funding partners and maximizes billing utilization. Ensure that the program provides meaningful, and quality services, meets participants' needs, ensures health and safety, and reaches desired outcomes.
* Ensure the timely assignment of referrals, accurate management of the authorizations, and completion of monthly billing processes.

Additional Responsibilities
* Serves as the primary FHF program point of contact to coordinate core services and team meetings (Housing Manager, Job Developer(s), and Volunteer Resident Advisors).
* Coordinates and manages Friendship House townhalls and/or newsletters.
* Coordinates with communications teams for social media highlights, Friend/Student Professional openings, and other marketing needs.
* Attend weekly meetings with Rehabilitation staff.
* Ensures program goals, individual roles, duties, and responsibilities are in sync with the mission of Friendship House in providing an intentional and transitional living experience.
* Develop and conduct consumer satisfaction surveys, analyze results, and recommend program/enhancements as needed.
* Training students/young professionals and staff on mentorship, disability sensitivity, and communication.
* Participate in internal and external conferences, audits, councils, committees, and town hall meetings.
* Perform other responsibilities as assigned.

Qualifications: Education, Experience, and Certification(s)
* Required bachelor's degree in related rehabilitation field and minimum of three years management experience working with individuals with intellectual/developmental disabilities.
* Must meet eligibility requirements of a Qualified Professional under Medicaid standards.
* Thorough understanding of Medicaid standards, NC licensure regulations, Human Rights, case note documentation, and ADL/IADL assessments.
* Preferred post graduate knowledge in a related rehabilitation field, e.g., master's degree in social work.
* Preferred experience in supportive housing, intentional living communities, counseling, and therapeutic interventions.
* Must pass a criminal background check, drug testing and healthcare registry check.
* Must possess a valid North Carolina driver license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).

Knowledge, Skills and Abilities
* Must be able to work with people internally and externally at all levels of an organization.
* Proficiency in Microsoft Office skills, with the ability to become familiar with corporate specific programs and software.
* Excellent written and oral/verbal communication skills.
* Detail-oriented and able to carry out work with high levels of accuracy.
* Excellent Case Management skills and demonstrated skills of program evaluation.
* Demonstrate strong skills in interviewing, engagement, assessment, planning, and advocacy.
* Must be able to successfully manage & coordinate multiple responsibilities and simultaneous duties.
