Responsibilities
  • Patient Care & Treatment - Conduct in-home assessments, develop individualized care plans, and provide physical therapy interventions to restore function, relieve pain, and improve patient independence. Educate patients and caregivers on treatment plans, equipment use, and disease management.

  • Care Coordination & Documentation - Collaborate with physicians and interdisciplinary teams to ensure comprehensive patient care. Maintain accurate, timely documentation of assessments, care plans, progress notes, and physician communications to support high-quality outcomes.

  • Compliance & Quality Assurance - Adhere to home health regulations, safety protocols, and professional standards. Participate in quality improvement initiatives, training, and peer reviews while ensuring effective use of resources and proper supervision of support staff.


Qualifications

Education:

  • Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.
  • Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.

Experience:

  • One year experience caring for similar patient population as to be assigned.

License(s)/Certification(s):

  • Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
