Resource Nurse Care Manager providing assessment, care coordination and discharge planning for patients in pediatrics, Peds ICU and the Neonatal Intensive Care Unit. This position will partner with social work and work closely with the medical team. This role will fill in for various caseloads to help cover PTO, medical leaves and open positions. Minimum of 2 shifts per month but seeking someone with greater availability if possible. Scope of Work
Plans and prioritizes care for individuals and population of patients, focusing on strategies that will promote optimal health within populations. Demonstrates expertise, current knowledge in nursing care and management of a caseload of patients of varying complexity and seeks to improve patient, family and health systems / community outcomes through the application of educational concepts / skills and preventive care in a managed care environment.
