About the Role: Are you passionate about making a real difference in the lives of families in need? As a Care Coordinator (Wraparound Facilitator), you'll be at the heart of our mission, helping adolescents (ages 5-21) and their families navigate challenges and build stronger, healthier futures. Your role will involve building and working closely with a team to create personalized support plans and coordinate a range of services that make a meaningful impact. What You'll Do: Connect with Families and Team Members: Reach out to new families within 3 days and meet with them face-to-face within 7 days to start building strong relationships. Lead the Way: Build a team of supports and facilitate monthly Child and Family Team Meetings (CFTMs), keeping everyone updated with an evolving Plan of Care. Create and Customize: Develop and monitor personalized Plans of Care, ensuring they meet each family's unique needs and goals. Coordinate with Care: Make sure families get the educational, social, and mental health support they need, and ensure these services are high-quality and accessible. Keep It All on Track: Maintain detailed and timely records, including updates in our Electronic Health Record System. Safeguard Privacy: Handle all information with the utmost confidentiality, adhering to HIPAA, FERPA, and other privacy standards. Who We're Looking For: Education & Experience: You have a bachelor's degree in a human services field (or a related field with at least 2 years of experience in family, children/youth, or community services). If you have an associate's degree and relevant experience, we're open to discussing your application! Training: You're ready to complete the required training and credentialing for Wraparound care coordination. Skills & Qualities: Strong communicator, organized, and great with people. Your enthusiasm for helping families shines through in everything you do. Practical Needs: Reliable transportation, a valid driver's license, and auto insurance are essential. Why Join Us? According to numerous independent studies, most youths in welfare programs that interacted with Ascent move on to less restrictive environments and are significantly less likely to engage in illegal or delinquent behavior.
