Advanced Practice Provider (PA/NP) - Cardiology, Advanced Heart Failure

Position Description:

The Medical College of Wisconsin's Department of Medicine, Division of Cardiology is seeking a Nurse Practitioner or Physician Assistant to provide primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures. located in Milwaukee, WI at Froedtert Hospital, Center for Advanced Care and the HUB.

The Advanced Practice Provider (APP) works as part of a multidisciplinary health care team, providing a broad range of diagnostic and therapeutic services for the patients primarily in the outpatient setting as a member of the Department of Medicine - Advanced Heart Failure, Mechanical Circulatory Support and Transplant Cardiology Program (AHF-TX) in the Division of Cardiology).

In this role you will be responsible for the following:

  • Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring medical intervention in blended environment as a member of the Advanced Heart Failure and Transplant Cardiology Program (AHF-TX).
  • Writes accurate and timely progress notes, procedure notes, and/or admission and transfer orders.
  • Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information.
  • Privileged to perform procedures beyond core as identified by the department.
  • Facilitates and communicates with patient, family and staff to promote continuity of care across the continuum of care.
  • Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission.
  • Evaluates quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and VAD/cardiac transplant outcomes dashboard.
  • Acquires information on the most current development and trends in the Physician Assistant and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature.
  • Attends departmental, divisional and programmatic meetings.
  • Works collaboratively with the advanced heart failure and surgical teams to provide complex, inpatient care related to heart failure, VAD and cardiac transplant patients.
  • Outpatient APP clinics: 8 half-days per week on rotation.
  • After hour call coverage for complex specialty patients on the inpatient floor patients, Emergency Department, Access Center for patient triage needs and assisting in management of pre cardiac transplant orders and coordination of care.

Core Advanced Practice Responsibilities:

  • Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.
  • Orders and interprets laboratory and diagnostic tests.
  • Develops pharmaceutical and non-pharmaceutical treatment plans.
  • Monitors effectiveness of plan of care.
  • Educates patients on disease and treatment plans.
  • Performs procedures as trained and appropriately credentialed.
  • Documents within the Electronic Medical Record (EMR) and completes other necessary documents.
  • Collaborates and/or consults with physicians and other health care providers as appropriate.
  • Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.

Preferred Schedule: Scheduling flexibility required to provide for an appropriate level of patient care coverage. Full time opportunity. Rotation with inpatient service and APP outpatient independent clinics (fulfilling 8 half days). Built in administration time per clinic week, according to the DOM guidelines within this schedule. There is a 24/7 call rotation (home based), weekday overnight. Call coverage intermittently on weekends and holidays including MCW observed holidays.

Preferred Experience: Two years with an advanced heart failure program as an RN or APP or two years of experience within heart failure or general cardiology.

Minimum Education:

Advanced Practice Nurse: Master's Degree or higher Nurse Practitioner program
Physician Assistant: Bachelor's Degree or higher in Physician Assistant Studies
