Summary
Responsible for patient care and support within our hospital's outpatient setting through providing compassionate care, assisting in procedures, and collaborating effectively in a dynamic ambulatory care environment.
Essential Functions
*Deliver direct patient care and support in our ambulatory care unit, ensuring high nursing standards.
*Conduct patient assessments, devise care plans, and assist in outpatient procedures with a focus on patient comfort and safety.
*Educate patients and families on treatment plans, medications, and post-procedure care.
*Collaborate closely with healthcare professionals to coordinate comprehensive patient care.
*Administer medications, perform IV therapy, and maintain accurate patient records in compliance with regulations.
*Uphold infection control protocols and maintain a safe environment for patients and staff.

To qualify you must have: Education:
Experience:
1. Minimum of 2 years of recent (within 12 months) clinical nursing experience in an Ambulatory Clinic/Practice setting and/or emergency room setting required. Experience only in an ambulatory procedural area not sufficient. 2. Experience providing in-person triage and direct patient care for acute and chronic conditions as appropriate for an ambulatory outpatient setting, urgent care, or ED required. 3. Experience with telephone triage and telephone-based clinical assessmenttechniques required. This is the safe, effective, and appropriate disposition of health-related problems and includes:assessing the severity of symptoms, communicating urgent patient needs to the LIP, referral to appropriate setting or level of care, documenting interventions, and an individualized plan of care using an EMR, education of patients and families on their disease process and interventions to promote health and well-being, preliminary interpretation of data, providing home health advice and follow-up, ability to make triage decisions quickly and calmly in emergency situations, and assuring continuity of care. 4. Experience with Endocrine, Renal andGastroenterology-related patient care delivery as an RN strongly preferred. 5. Experience with team-based care preferred. 6. Case Management experience preferred.
