Job Description:
- Health District is seeking to hire an individual to work with our community-based team to ensure greater equity and access to community resources and vaccines. We are interested in self-motivated individuals who have a desire to make a positive impact in the community. The Community Health Worker will connect with community members, advocate in favor of vaccine adoption, work to remove barriers and connect residents to appropriate resources. This position will allocate time across all counties in the district, with a home base in Lynchburg. Telework is available for up to 12 hours per week.
Role & Responsibilities:
- Develop and sustain relationships with community-based organizations addressing food insecurity, nutrition, and vaccine access within the assigned area.
- Assist in planning, executing, and evaluating activities related to food security, nutrition education, and vaccine promotion, including in-person outreach, resource connections, and participation in community events.
- Gather, analyze, and report community feedback regarding food insecurity, nutrition barriers, and vaccine hesitancy.
- Interpret and apply data to inform initiatives targeting both food access and vaccine promotion strategies.
- Collaborate with internal and external partners to create project activity plans focused on improving food security, nutrition, and vaccine awareness.
- Stay informed about local and national resources related to food insecurity, nutrition, and the latest vaccine guidance and best practices for prevention.
- Utilize Unite Us and other tools to connect community members with food resources and vaccination opportunities and ensure proper follow-up.
- Brainstorm creative outreach methods to raise awareness and share information about nutrition, food resources, and vaccination.
- Work closely with the Population Health team to address food insecurity and promote vaccines, while taking on other assigned tasks or responsibilities.
Minimum Qualifications:
- High School diploma.
- Reliable transportation-there is frequent travel within the district.
- Knowledge of the local health district and resources available.
- Ability to explain health information to the public in a clear, concise, and accessible manner.
- Strong interpersonal skills, including the ability to relate positively to staff members and the public.
- Ability to analyze, report, and apply data to ongoing initiatives.
- Strong oral and written communication skills and organizational skills.
- Working knowledge of Microsoft Word, Excel, Outlook, and the ability to learn other software as needed.
Preferred Qualifications
- Associate degree or above in a health-related field.
- Resides in the Central Virginia Health District.
- Desire to become a Certified Community Health Worker.
- Experience performing community outreach and collaborating with community-based organizations.
Remarks:
- Full-time, 35-40 hours per week (workday/times may vary to cover after hours and weekends when needed).
