Overview The Triage Associate will assess new patient referrals for appropriate level of mental health care. Performs phone screenings and in-person assessments and arranges/recommends follow-up care. Provides short-term, solution-focused counseling to a subset of patients Qualifications Certifications: Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR)Illinois Department of Financial and Professional Regulation (IDFPR); Licensed Professional Counselor (LPC) - Illinois Department of Financial and Professional Regulation (IDFPR)Illinois Department of Financial and Professional Regulation (IDFPR), Education: Master's Degree: Social Work; Master's Degree: Related Field, Work Experience: Triage services Responsibilities Assesses new patient referrals. Determines the appropriate level of mental health care based upon phone screen (outpatient psychotherapy, medication evaluation/management) based on phone screening and/or in-person assessment. Provides short-term, solution focused counseling to a subset of patients. Provides education to patients, families and group Documents accurately and completely. Screens new patients referred to the Psychiatry/Psychology Department for treatment, and refers new patients for appropriate services inside and outside the Department. Completes appropriate documentation in medical record within time frames established by departmental policy. With permission of patient, provides communication back to referring providers about patient diagnosis, treatment plan, and progress Maintains patient confidentiality and fulfills provider responsibilities as specified under HIPAA, the Illinois Mental Health and Developmental Disabilities Code, and the Illinois Mental Health and Developmental Disabilities Confidentiality Act. Compensation and Benefits The compensation range for this position is $23.12per hour - $38.61per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
