Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

Providing meaningful support to prospective patients by processing incoming referrals and scheduling clinical visits. Coordinate patient visits with clinician schedules and support mileage tracking to create efficiencies. Create positive impact on payor client engagement goals and grow enrollment numbers through targeted outreach, enrollment, and scheduling.

Responsibilities include:



  • Data entry for patient admissions and provider referrals.
  • Handle outreach and inbound calls (via telephone) to patients to enroll them with our services and schedule visits.
  • Correspond with payor clients regarding the status of patient engagement and scheduled visits.
  • Create clinician schedules using mapping tools.
  • Communicate with clinicians providing visits regarding scheduling updates.
  • Track clinician travel mileage.
  • Collaborate with other departments on patient care coordination.
  • Prepare reports for management and payor clients.
  • Actively support the achievement of SCAN's Vision and Goals.
  • Other duties as assigned.


Qualifications and experience:



  • High School Education or GED, preferred but not required

  • 6 months to 1 year of relevant scheduling experience.


  • Bilingual Spanish required.

  • Experience within specific department area, preferred.



  • Strong interpersonal skills, including excellent written and verbal communication skills.



  • Strong organizational skills.



  • Ability to handle multiple tasks simultaneously and work independently with minimal supervision.



  • Ability to manage and maintain confidential information.
