Summary/Description:

The UIC Retzky College of Pharmacy, Department of Pharmacy Practice is seeking a full time, motivated professional for the position of Clinical Faculty (Open Rank)/Clinical Pharmacist who will be assigned to UI Health's Solid Organ Transplant Service.

The ideal candidate will be responsible for providing pharmaceutical care to patients in the area of solid organ transplant in the hospital and on-site clinics at the University of Illinois Hospital. The hospital is a 450 bed academic institution located approximately 2 miles west of downtown Chicago. The hospital serves an urban, indigent patient population. The UIC Transplant Center is a renowned program offering state-of-the-art kidney, liver, kidney-pancreas, isolated pancreas, small bowel, and islet cell transplantation services.

Duties:
The current clinical pharmacist team consists of 8 pharmacists who are integral members of the multidisciplinary team. Transplant Pharmacists have been members of UI Health transplant team for over 40 years. They provide comprehensive medication therapy management to all transplant patients (donors and recipients) in both inpatient and outpatient settings. In 2010, the role was expanded to include evaluation of all transplant candidates. Since 2015, transplant pharmacists at UIHealth practice under the Comprehensive Medication Therapy Management Protocol.

Clinical Pharmacist duties include medication review and reconciliation, medication and dosing recommendations, immunosuppression and infectious disease prophylaxis selection, post-operative management of blood glucose, blood pressure, nausea and pain, therapeutic drug monitoring, provision of drug information, and patient education. The candidate will also be responsible for nurse, physician, and pharmacy staff development; coordinating and conducting quality initiative projects and developing collaborative treatment and research protocols.

The academic responsibilities of the clinical faculty member (Clinical Assistant/Associate/Full Professor) include the provision of didactic lectures and precepting entry level Doctor of Pharmacy students, PGY1 residents and PGY2 residents. Scholarly activities, including presentations and publications, are an expectation as well.

Minimum Qualifications
PharmD degree from an ACPE accredited Doctor of Pharmacy Program.
Completion of Post Graduate Year 1 (PGY1) pharmacy residency required.
Completion of Post Graduate Year 2 (PGY2) solid organ transplant pharmacy residency or 2-3 years of equivalent clinical experience as a clinical pharmacist in solid organ transplant is required.
Must present evidence of, or eligibility for, Illinois licensure as a professional pharmacist by start date.
Must possess knowledge and skills such that they qualify for a non-tenured clinical faculty position within the Department of Pharmacy Practice at the UIC College of Pharmacy. Academic rank commensurate with experience.

Highly preferred: strong clinical knowledge base; analytical and problem-solving skills; strong verbal and written communication skills; ability to consistently meet deadlines; demonstrate flexibility and ability to work independently and adapt to a changing work environment; excellent planning, time management and organizational skills.
