Job Description

The RN- Nurse Navigator acts as a guide and advocate for patients diagnosed with cancer within the Baptist Cancer Institute and Baptist Healthcare. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of cancer patients to facilitate achievement of quality and cost-efficient patient outcomes from screening to survivorship. This role assists the patient throughout their care continuum to ensure that proper acute and supportive treatment is provided in a consistent and timely manner by focusing on the patient and family while ensuring the patient's active participation and decision making in their care. The role guides patients through our comprehensive healthcare system works with physicians and multidisciplinary team members to further diagnose or treat, provide adjunctive therapy, coordinate and maintain patient appointments and answer patient's questions, according to the agreed-upon clinical guidelines, to coordinate high-quality patient care across all care delivery sites. The Nurse Navigator-Oncology ensures timely access to healthcare services and provides patient information and education, as needed, to ensure the best patient healthcare experience possible. The Nurse Navigator will manage and assist with any accreditation standards as well as program-related analytics as assigned by the manager. The Nurse Navigator also supports and helps execute specific Baptist Cancer Institute initiatives as directed by the Cancer Support Services Senior Manager as directed.

Responsibilities

  • Assesses and diagnoses patient and family need to provide quality care to assigned patients.
  • Develops, discusses, and communicates a realistic plan of care for each patient in order to address all identified needs.
  • Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions, and procedures as necessary for the care of the patient.
  • Evaluates the identified problems, care provided, and patient's responses are reassessed to meet patient and family needs and assure overall quality of care delivered.
  • Maintains up to date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team, to ensure quality care.
  • Develops and sustains excellent relationships and communications with our primary care physicians, APPs, practice managers and staff. Ensures that care coordination and population health management systems meet the needs of the patients and our primary care providers.
  • Documents all communication and coordination activities in the patient medical record to ensure transitional care billing requirements are met.
  • Develops knowledge of and relationship with community health services providers to ensure primary care patients needing community services (food, transportation, childcare, etc) are able to access those services.
  • Works with appropriate in-patient and long-term care departments and staff to develop and maintain excellent transitions of care processes.
  • Develops systems and processes to support delivery of and coding of Transitions of Care Management and Chronic Care Management.
  • Manages the continuum of care for the oncology patient throughout their entire treatment course in conjunction with Commission on Cancer standards and guidelines.
  • Supports Baptist Cancer Institute initiatives as directed and utilizes system support services to deliver comprehensive care to the patients and families we serve.
  • Supports the successful processes needed to maintain Commission on Cancer accreditation, while assisting to provide highest quality outcomes and assurance measures for cancer patients served.
  • Maintains current knowledge/certification/licensure. Pursues professional growth and development.


Qualifications

Minimum Education

  • Technical Diploma/Certificate Nursing Required or
  • Associates Degree Nursing Required or
  • Bachelor's Degree Nursing Preferred

Minimum Work Experience

  • 4-6 years clinical based nursing or clinical case management Required
  • 2 years experience in nurse navigation or care coordination Preferred

Licenses and Certifications

  • Registered Nurse Licensed State of Florida or eligible compact Upon Hire Required
  • BLS for Healthcare Providers (BLS) Upon Hire Required

Required Skills, Knowledge and Abilities

  • Demonstrated skills in the areas of negotiation, communication (verbal/written), conflict, interdisciplinary collaboration, management, creative problem solving, and critical thinking.
  • Knowledge of healthcare financing, community and organizational resources, patient care processes, and data analysis.
  • Excellent verbal and written communication skills.
  • Demonstrates flexibility via an ability to adapt to changing priorities and regulations.
  • Basic computer skills.
