Job Summary:
The Care Coordinator is responsible for providing care coordination to patients and families, helping to navigate complex care systems including home, community, school, and mental health. The Care Coordinator collaborates with an interdisciplinary care team (which may include the patient/parent, primary care provider, specialty providers, case managers, school team members, etc.) to coordinate care plans, provide education and support, and connect families with appropriate resources.

Essential Duties and Responsibilities :

  • Provide care coordination services to patients/parents, which includes building care relationships with families and conducting regular patient outreach and follow up.
  • Assess patient and family needs and potential barriers to determine appropriate referrals and resources. Research, facilitate, and navigate resources and services to medical, behavioral health, county, and community providers and organizations as needed.
  • Develop relationships with community providers and agencies.
  • Collaborate, coordinate, and provide timely communication with the patient care team.
  • Document all care coordination activities in the patient's electronic medical record (EMR).
  • Daily activities may include scheduling appointments, document management, patient portal messaging, reporting and data analytics, working with interpreters, assisting with transportation needs, etc.
  • Responsible for registry management.
  • Other duties as assigned.


Education / Credentials / Qualifications:

  • High school graduate or GED. Associate's or Bachelor's degree preferred. Areas of educational preparation or equivalent experience may include but not limited to, social work, psychology, mental health, nursing, public health, community health worker, or other related fields of study.
  • 2+ years of experience in a healthcare setting preferred.
  • Prior experience in care coordination, social work, case management, clinical, or community health setting preferred. Experience with managing populations with complex medical or behavioral needs preferred.

Hours are between 8:00am - 5:00pm M-F. Travel between clinic locations required on occasion.
