Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

The Community Health Worker supports palliative care program members by providing physical, emotional, and social support via ongoing in-person, televideo or telephonic visits. Assists with coordination of services and collaborates with healthcare professionals, community organizations, and families to ensure comprehensive holistic care for patients.

This role will support a patient caseload from Homebase's Palliative Care Program, as well as support clinical patients who reside in Ventura County and need assistance with SDoH, Caregiver Support and other factors impacting health and health outcomes. This is a mobile/field position.

Responsibilities include:



  • Assesses patients' home environment to identify physical, social, emotional, spiritual, and knowledge barriers to optimal care; reviews adherence to physicians' recommendations. Maintains accurate and up-to-date documentation of patient interactions, care plans, interventions and outcomes. Provides regular reports to the healthcare team on patients progress and any changes in their condition. Facilitates communication and information sharing between the patient, family, and healthcare team.
  • Promotes patient autonomy and informed decision making. Facilitates patient's health through advocacy, assists with appointment scheduling, arranging transportation, advance care planning, and coordinating of medication management.
  • Assists patients and caregivers in taking an active role in health management and promotion through coaching and education. Assists families in understanding and implementing caregiving responsibilities and with the development of a support system.
  • Supports with understanding and use of benefits, community and public resources; actively assists patients and caregivers to secure resources or use of benefits. Assist with navigating the healthcare system.
  • Exhibits strong interpersonal, critical thinking, analytical/organizational skills, decision-making, prioritization, multi-tasking, and problem solving to achieve successful outcomes.
  • Delivers health information using culturally appropriate terms and concepts.
  • Enhances skills and knowledge by participating in educational offerings, team case conferences, and training per departmental guidelines.
  • Adheres to department desktop procedures, workflows, job aids, and training material. Identifies barriers to work process and brings to the attention of supervisor/manager for process review and improvement.
  • Maintains the patient's right to privacy and protects both SCAN and Medical Group operations by keeping information confidential.
  • Adheres to all quality, compliance, and regulatory standards to achieve desired outcomes.
  • Contributes to team effort by accomplishing related results as needed.
  • Ability to drive to patient homes in assigned area required.
  • Actively support the achievement of Homebase's Vision and Goals.
  • Other duties as assigned


Qualifications and experience:



  • High school diploma or equivalent
  • Associate's degree or certification in community health or related field preferred
  • 5+ years of experience in healthcare, or proven experience working closely with diverse communities.
  • Fluent in Spanish, required.
  • Cultural sensitivities and awareness of the social determinants of health.
  • Ability to work independently and as part of a team, with strong organizational and time management skills.
  • Ability to communicate with clarity, the vision, value, and relay health information to individuals and groups.
  • Able to develop relationships quickly and leverage working relationships to drive performance outcomes.
  • Strong interpersonal skills, including excellent written and verbal communication skills.
  • Strong organizational skills; Ability to multitask and to appropriately maintain confidentiality and adherence to ethical standards.
  • Requires proof of Valid Driver's License
  • Must maintain State Liability Insurance
  • Must have access to a Vehicle
