The Community Health Worker (CHW) reports to the Department of Pediatric Social Work. Ideal candidate will focus on supporting uninsured and/or underinsured pediatric patients in Baltimore, MD region with chronic medical conditions who are treated within the Johns Hopkins Children's Center in both inpatient and outpatient settings. The CHW will assist families of patients with enrolling/ maintaining enrollment in MDH's Children's Medical Services (CMS). The CHW will provide care coordination to the family, assisting with scheduling specialty appointments, coordinating the need for medical equipment and medications, and submitting insurance prior authorizations for necessary care.

CHW will communicate with families in person and over the phone to complete necessary applications and authorizations for Children's Medical Services and provide medical care coordination as needed. CHW will complete home visits as necessary and attend appointments with clients within the community agencies to ensure the completion of the CMS application. The CHW I must have reliable daily transportation for work related travel (to and from community site, patient homes, meetings, clinics, etc.). If using personal vehicle for work related travel, the CHW must have appropriate license and insurance.

Work Location: This is a hybrid position with the expectation that the candidate will work onsite (Baltimore, MD) for 2-3 days per week. During the remainder of the week, visits to member's homes, clinic sites, or community organizations will be required. This may involve exposure to environments outside of the control of Johns Hopkins

Requirements:

Education:




  • High school diploma or equivalent required.
  • Must be able to speak, read and write in English. Bilingual (Spanish/ English candidate required.)



Licensure/Certification Required:




  • Successful completion of the Maryland state CHW certification program. (or completion within first year of hire)



Knowledge:




  • New employees must complete all required training within 30 days of hire unless otherwise noted.
  • Familiarity with resources in the Baltimore region related to health and social stabilization strongly preferred. Ability to proactively outreach to patients and families to address barriers to accessing healthcare
  • Preferred Years of Experience: 1-3 years experience
  • Requires working knowledge of social and health issues.
  • Familiarity with medical terminology beneficial



Salary Range: Minimum 19.92 per hour - Maximum 32.89 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
