Scope of Position/Position Summary: The Patient Care Resource Manager, PCRM, facilitates patients through the health care system to include pre-admission to post discharge coordination of care. This may involve both the inpatient and outpatient settings. The PCRM monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive health assessment, treatment, and follow-up evaluation for patients. This includes health risk appraisal, health promotion, and clinical research. The PCRM is a primary educator for the payers, providers, and the public and serves as the central information source for third party payers. The PCRM maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization. The PCRM works closely with other PCRMs on the service and they provide coverage for each other as needed. Minimum Education Required Bachelor's Level Degree or equivalent combination of education and experience with a Major in nursing Required Qualifications Bachelors degree, Masters preferred. RN or other professional healthcare licensure; five years recent clinical experience applicable to the population preferred; knowledge of clinical practice guidelines, computer data bases, and payer protocols desired.
