Summary:

The Advanced Practice Provider reports to the Director of the Cardiovascular Institute. Responsible for the planning direction and operation for clinical aspects of the patient coordination for the Cardiac Surgery Program. In collaboration with the MD provides clinical support to drive cardiac surgery efficiency from consult to procedure. This will include but not limited to screening education consultation and coordination of care from the pre-procedure through procedural recovery.

Responsibilities:

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Primary responsibility is for Cardiac Surgery but will include some aspects of the Structural Heart Program. Responsibilities will include autonomous management of all aspects of pre- and post-operative care. Coordinates the daily operational activities required for these programs. Responsibilities will include but not limited to working with the surgeons to navigate patients thru the process from home outside hospitals and inpatient by performing history and physicals expediting workups and collaborating with the surgeons to efficiently drive patients through the system. Will also include pre-admission testing sick visits outpatient calls and discharge clinic.

In collaboration with physicians conducts assessment of patient needs by evaluating/interviewing all new /returning patients. Triages patients to appropriate care resource (includes direct contact and telephone encounters). Reviews patient history and other supporting clinical data to facilitate optimum care.

Acts as a liaison between RIH and outside hospitals to navigate patient coordination to RIH. Will also be responsible to navigate coordination for Cardiac Surgery/Structural Heart Programs within the Cardiovascular Institute. Performs all pre- and post-procedure follow up support to patients and physicians. Works with physician and administrative CVI leadership to develop implement and track relevant quality outcome and patient satisfaction measures.

As an advanced practice provider will work with the physician team to decrease care gap and facilitate throughput. The APP will work with primary inpatient teams on all OSH transfers direct and readmits. Communicates the plan of care and other pertinent information to/from other healthcare providers.

Will also work as a liaison for the office staff for pre- and post- procedure issues.

Will work with our clinical research nurses to support cardiac surgery and structural heart research requirements.

Collaborates as a skilled communicator working among multiple departments simultaneously. Demonstrates ability to be highly organized with foresight for prioritization and action. Ensures compliance and achievement of Hospital wide Quality Improvement Plan. Works with the physician and administrative CVI leadership to regularly update and identify appropriate quality improvement metrics. Oversees ongoing quality assurance activities by collecting measuring and analyzing statistical data collects and reports outcomes (internal/external) including but not limited to length of stay patient satisfaction operational and functional measures and others as requested.

Assists with community outreach and awareness efforts.

May perform other duties as requested by staff physicians or attending physicians including care of selected patients on both the medical and surgical services and initiating consultative services on behalf of the CVT surgery service.

BASIC KNOWLEDGE:

Physician Assistants must be graduates of an accredited PA program with current NCCPA certification.

Nurse Practitioners must be graduates of a Nurse Practitioner program and certified by the ANCC or by the AANP.

Candidates should hold or be eligible for licensure in the state of Rhode Island as a Physician Assistant or Nurse Practitioner eligible for licensure for Federal DEA certificate. Completed approved BLS/ACLS courses.

EXPERIENCE:

Ideal candidates would have some cardiology or cardiac surgery experience but highly qualified new graduates will be considered. On-the-job training and experience to become familiar with job responsibilities and work routines will be provided. All applicants should possess a high level of ability to function with significant clinical autonomy.
