The Ryan White HIV Clinic seeks an HIV Medical Case Manager Supervisor. The Case Management Supervisor is a key member of the leadership team at the federally-funded UVA Ryan White Clinic. This person oversees the team of individuals who provide comprehensive psychosocial support services to our patients. This person is responsible for supervising the Ryan White Program Case Management team including, Medical Case managers, Non-Medical Case Managers, Community Health workers, and Housing Coordinator. He/She will supervise multiple staff members and will directly be involved in decision-making for hiring, Performance Management, and discipline. He/She will ensure that the Case Management office is staffed appropriately and that daily operations operate according to the UVA Health System and Ryan White Program procedures. The Case Management Supervisor will also coordinate the patient caseload assignments, the day-to-day delivery of patient care coordination, and case management services. In addition, he or she will make sure that the staff are working with patients to develop income-based case plans that address their barriers to securing and maintaining safe stable housing. Key Performance Area: Patient Care
Works in collaboration with the Medical Providers in the ID clinic to identify clients at intake that are in need of Medical Case Management. Collaborate and Consult with Clinical Staff to identify current clients who are in need of medical case management services. Respond within 24 hours to requests for case management services. Complete patient screening to determine eligibility for medical case management services. Provide feedback to patient's provider care team in the ID clinic as needed.
Key Performance Area: Staff Supervision
Oversee the clinical and administrative operations of the Medical Case Management, Non-Medical Case Management, and Community Health workers Services. Provide regular supervision to case management staff with diverse areas of expertise including job development. Supervising towards LCSW if applicable. Ensure that Case Managers continually monitor and evaluate each patient's progression through their case management plan, developing corrective action revisions to the plan as needed. Ensure the timely flow of Case Management activities to meet program goals and objectives. Oversee the integration of trauma-informed and gender-responsive services for men and women participants. Use motivational interviewing and supervise staff in use of motivational interviewing to engage and retain patients in services. Allocate time to direct patient care services as a member of the clinical team, including direct work in the community and with staff as needed. Assess patient immediate needs, legal circumstances, and engagement needs to develop service plans. Complete assessments, progress notes and significant program data. Delegate and assign responsibilities to Case Management office staff and assess employee performance as needed. Develop and implement office policies and procedures, helping to foster a culture of compliance. Formulate objectives and processes for the Case Management Team to improve efficiency and ensure that daily operations run smoothly. Complete required grant reporting timely and collaborate with others on the team on developing grant reports. In addition to the above job responsibilities, other duties may be assigned.
Key Performance Area: Continued Contact
Face to face interaction with client at least every 90 days. Complete review of Service Coordination Plan every 90 days; making any changes, additions or deletions to current services. Obtain clients signature on Service Coordination Plan review indicating the agreement for continued contact and case management services. Develop intervention plan to re-engage client if face-to-face contact is not maintained and include the intervention in the Service Coordination Plan. Document treatment adherence activities in clients file; these include keeping medical appointments, taking prescribed medication, refilling prescriptions, etc. Complete reassessment annually including the development of a new Service Coordination Plan.
Training and other Requirements:
VAMAP requirements and applications. Medicaid, Medicare, SSI and SSDI requirements. HIV Case Management Standards. HIV 101: including diseases process, treatment, testing. Legal issues such as confidentiality, counseling/referral and prevention. Cultural Competency. RW Eligibility requirements. Motivational Interviewing training. Participates in staff trainings and in-services at VDH. Participates in opportunities offered by the MidAtlantic AIDS Education and Training Center and Virginia HIV/AIDS Resource and Consultation Center. Participate in educational training provided by local agencies in regarding to HIV care or services needed by clients. 12 hours of continuing education is required for Medical Case Manager.
MINIMUM REQUIREMENTS: Education: Bachelor's degree or equivalent experience in a human service field. Experience: 1-2 years Case Management experience preferred. Prior working experience with the HIV/AIDS community is preferred. Licensure: LCSW Preferred but not Required. Preferred Requirements: Education, experience, and licensure (one of three options)
Master's Degree in Social Work, Psychology, Counseling, Education or Sociology and at least four years of increasingly responsible employment in related field is preferred. LCSW licensure preferred. Management experience in social service setting with working knowledge of Case Management systems and planning techniques. At least ten years of administrative experience in a health care environment is preferred. Knowledge of substance abuse recovery and mental health care necessary. Experience working with homeless individuals and families a plus. Experience in administering public funding is desirable.
PHYSICAL DEMANDS This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
