Job Summary

Provides supportive services to patients of any age and their families to meet spiritual, social and emotional needs prior to and after the death.

Essential Functions

  • All team members are expected to be knowledgeable and compliant with Prisma Health's values: Inspire health. Serve with compassion. Be the difference.

  • Initiates interview with the patient/family within 48 hours of admission to Hospice and assesses spiritual needs and spiritual care resources.

  • Plans/Offers age-appropriate spiritual counseling, based upon knowledge of the stages of development and end of life issues: Pediatrics, Adult, Geriatrics.

  • Plans/Offers spiritual or bereavement counseling and support for clients of all ages, including hospice staff, based upon a knowledge of the different ways in which each person processes grief and loss (Pediatrics, Adult, Geriatrics).

  • Participates in IDG care planning, offering insights and knowledge to other team members in a supportive manner.

  • Evaluates the effect of spiritual concerns on the overall comfort of the patient.

  • Explains the hospice POC process and invites the minister to attend IDG.

  • Carries out the bereavement plan (letters, memorials, counseling, support group, etc.) in a timely manner and documents all activities and contacts in the bereavement record, as assigned.

  • Cultivates an ongoing relationship with the religious community in order to educate them and develop support for hospice.

  • Educates the public regarding end-of-life issues.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Master's degree of Divinity or related field of study.

  • Experience - Five (5) years of pastoral experience. Congregational setting preferred

Required Certifications, Registrations, Licenses

  • SC Driver's License - Current driver's license and automobile insurance.

  • Completion of Clinical Pastoral Education Units (CPE).

  • Board Certified Chaplain by the Association for Professional Chaplains (APC, Inc.) - Preferred

  • Board Certified Eligible by the Association for Professional Chaplains (APC, Inc.) - Preferred

Knowledge, Skills and Abilities

  • Knowledge of office equipment (fax/copier)

  • Proficient computer skills (word processing, spreadsheets, database)

  • Data entry skills
