Overview Job Summary The Guideline-Directed Medical Therapy (GDMT) program under the Advanced Heart Failure team is aimed to support patients with a reduced ejection fraction optimized on medications to treat decreased heart function and improve symptoms and heart function. Serves as a key member of the health care team and is a primary resource for assisting patients in navigating the care system. Reaches out to patients to provide support in the patient's adherence to their individual care plan, assisting to identify and remove any logistical, emotional or social barriers to treatment. Serves as the patients' primary conduit to the health care provider as they help coordinate necessary services both within and outside of the health system. Essential Functions
Physical Requirements Work may occur in a variety of settings, including office, clinic, school, or other community setting. Requires walking, standing, sitting, lifting, reaching, bending, pushing and pulling. Must be able to lift and support the weight of 50 pounds in handling equipment and supplies. Must have intact sense of sight, hearing, smell, touch and finger dexterity. Must be able to move about the medical center and to travel to various clinical and community locations. Education, Experience and Certifications High School diploma or equivalent required. Additional responsibilities as needed including but not limited to: data tracking, rescheduling labs, grant support, prior authorizations, and patient assistance programs. |
