Hourly Rate: $22.66

Must be willing and able to work in a correctional facility providing one-on-one, group instruction, and case management services

Duties & Responsibilities

  • The Reentry Facility Case Manager is the primary point of contact with the participants and others in the detention facility. The Reentry Facility Case Manager will recruit and enroll program participants, facilitate groups, and conduct one-on-one case management. The Reentry Facility Case Manager must display initiative, exercise judgment, and make decisions that are consistent with program goals
  • Confirms eligibility and manages enrollment activities, including scheduling and facilitating information and intake sessions in coordination with the Program Manager and Regional Manager
  • Creates and maintains an environment that reinforces appropriate behavior and positive morale; evaluates and monitors participant progress on work assignments; develops and uses predetermined daily and weekly lesson/work plans.
  • Assists participants with NCWorks registration, job readiness, resume development, IDP development, skills training, self-help and social skills, and using computers and cell phones.
  • Plans, implements, and coordinates events to increase success of participants to obtain and retain employment (such as mock interviews, job fairs, Virtual interviewing, and training and certification activities). Adapts materials and equipment to meet specific needs and learning styles of the population.
  • Monitors release dates and coordinated reentry transition planning in coordination with the Program Manager and Community Case Manager,
  • Maintains accurate Grantee Performance Management System data, including case records detailing enrollment, services, transition needs and referrals; ensures accurate and complete data entry according to established deadlines, while maintaining safety and integrity of participant information

Qualifications

  • Minimum Associate's Degree in combination with experience including lived experience; or Bachelor's Degree in Criminal Justice, Social Work, or related field (preferred).
  • 3+ years of full-time experience working with the justice-involved, peer support, or project management preferred.
  • Must be able to meet requirements for Eckerd Auto Insurance and drive for business purposes. Must have appropriate and valid state driver's license.
  • Required to pass a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy.
