With locations in Portsmouth and Norfolk, Sentara PACE recognizes that older people often have medical problems that last for long periods of time and require services from several different sources. Of great concern is the fact that, as many people age, they become increasingly isolated from the support, companionship and resources of the community at large. Sentara PACE is that important link, assuring participants receive the care they need, when they need it.

This program is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative for those who are eligible for a nursing facility but would prefer to remain at home.

The program provides total care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation. Sentara PACE is offered for residents of Norfolk, Virginia Beach, Chesapeake, Suffolk, Portsmouth, Surry, Isle of Wight and the city of Franklin.

Be a part of an excellent healthcare organization that cares about our People, Quality, Patient Safety, Service, and Integrity. Join a team that has a mission to improve health every day and a vision to be the healthcare choice of the communities that we serve!
