A Brief Overview

This position will be based out of the Emergency Department and has a focus on Substance Use Disorder. In this position you will collaborate with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment, educate and support to achieve long term healthcare outcomes.

Locations- Ahuja Medical Center (primary location) with travel to Cleveland Medical Center (Downtown CLE), Lake West Medical Center (Willoughby) and Tripoint Medical Center (Concord)

What You Will Do



  • Provide intensive case management to patients and assist with access to substance use and medical care, barriers in obtaining necessary resources; both community and insurance based.
  • Initiate and lead unbiased and trauma-informed conversations with any and all patients regarding their personal history of substance use.
  • Provide compassionate and professional conversation and care throughout complex patient conditions including during episodes of substance use.
  • Act as a guide in navigating transitions of care during a patient's health care journey.
  • Providing psychosocial assessment/intervention to assist patients/families (including clinically complex) in addressing barriers to receiving appropriate healthcare.
  • Monitoring and updating a comprehensive, complex care plan for individual patients based on identified psychosocial needs, discharge needs and overall population data.
  • Maintaining a current working knowledge of UH network and substance use services and promoting utilization of such services.
  • Effectively communicating and collaborating with the complex care team, and, where appropriate, patient's care providers, population health analysts, and community organizations using a multidisciplinary approach focused on improving the patients overall health and wellness.
  • Maintaining an extensive repository of UH and community resources to assist patients with psychosocial needs and where appropriate serving as liaison to community resources and agencies.
  • Provide follow-up and care within the community and patient home setting, as needed.
  • Identify and begin appropriate response to potentially life threatening conditions in patients seen in community or home settings.
  • Provide timely and professional ongoing data entry of programmatic data by identifying errors and inconsistencies of data in accordance with program guidelines.
  • Travel within Community/Home Visits

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
